A few procedural guidelines regarding policies and using your insurance

Insurance Information

I am a preferred provider for Blue Cross Blue Shield of Minnesota, at the Aware level. As part of my provider contract, I will submit claims on your behalf to your insurance. You will be responsible for payment of any copayment or deductible amounts at each appointment.

For all other insurance, payment in full is required at each appointment. As a Licensed Psychologist, I am covered by many insurers under the "out of network" portion of your policy and you may choose to submit a claim to obtain reimbursement. In order to do so, contact your insurance company to obtain a claim form, and I will be happy to provide you with a receipt to attach containing all information that is usually required.

Cancellation Policy

A 24-hour notification of appointment cancellation is required. You may leave a message at any time on my confidential voicemail. Please note that your appointment time is reserved for you, and will be charged at regular rates if notification is not received.
